- The form includes complete details of the seller, the vehicle and the buyer. Begin the form with the full and legal name (first, middle, last) of the seller. Then provide a detailed description of the vehicle to define its identity, which must include the year in which the vehicle was purchased, make of the vehicle and the Vehicle Identification number (VIN #).
- In the next part of the form, fill in complete details of the buyer such as their full and legal name (first, middle, last), their complete address along with the city, state and zip code. After that mention the dollar amount at which the vehicle is mutually decided to be sold. Below that also mention the complete address of the seller with their city, state and zip code.
- In the end, the seller has to sign the document certifying the deal and that the details mentioned above are true. This document also has to be notarized by the Notary Public for it to be considered legal.
